Ontwikkeling van houdingsregulatie tijdens externe perturbatie van het evenwicht bij kinderen met en zonder ontwikkelingsstoornissen (onderzoek wordt uitgevoerd in het Karolinska Hospital, Stockholm, Zweden en in de St. Mauritius Therapieklinik, Meerbusch, Duitsland)

Houdingsregulatie wordt bij kinderen met en zonder ontwikkelingsstoornissen onderzocht m.b.v. een bewegend platform. De kinderen zitten of staan op het platform dat plots naar voren of naar achteren beweegt; deze plotse beweging doet een duidelijk beroep op de houdingsregulatiecapaciteiten van het kind. Tijdens de perturbaties worden oppervlakte EMGs van nek-, romp- en beenspieren en kinematische signalen geregistreerd.
De onderzoeken toonden aan dat richtingsspecifieke houdingsactiviteit al bij jonge baby’s aanwezig is. Verder wordt houdingsactiviteit bij jonge baby’s gekenmerkt door variatie en het ontbreken van het vermogen om houdingsactiviteit te kunnen aanpassen aan de omstandigheden. Vanaf de leeftijd van 6 maanden begint het vermogen tot aanpassing van houdingsactiviteit zich heel geleidelijk aan te ontwikkelen.
Kinderen met ernstige vormen van CP (bijv GMSCF level V) missen het basale niveau van houdingsregulatie, d.w.z. het vermogen tot het genereren van richtingsspecifieke houdingsactiviteit geheel of gedeeltelijk. Kinderen met minder ernstige vormen van CP beschikken over het algemeen wel over richtingsspecifieke houdingsactiviteit. Zij hebben echter aanzienlijke moeilijkheden om de houdingsactiviteit precies aan te passen aan de omstandigheden.

Development op postural control during external perturbations in children with and without developmental disorders (studies carried out at the Karolinska Hospital, Stockholm, Sweden and the St. Mauritius Therapieklinik, Meerbusch, Germany)

Postural control is studied in children with and without CP by means of perturbations of a moving support surface. The children are studied in either a sitting or a standing position. Postural control is measured by means of multiple surface EMG recordings or neck-, trunk- and legmuscles and kinematics.
The studies showed that direction specific adjustments are present from early postnatal age onwards. Typical development is characterised by variable postural muscle activity which at early age can not be adapted to the specifics of the condition. From about 6 months onwards the ability to adapt postural activity emerges gradually. Children with severe forms of CP (eg GMSCF level V) totally or partially lack direction specific postural activity. Children with less severe forms of CP in general have direction specific activity. They do however have difficulties in adapting postural activity to the specifics of the situation.
